The light spectrum is very broad and is measured in nanometers. 1 to 20,000 nanometers to be exact. There is a tiny therapeutic window in the light spectrum where cold lasers work on mending human and animal cells. This window is between 400 and 1000 nanometers. Super pulsing is much different than a continuous wave laser. Because the super pulsed laser is constantly turning on and off, the delivery into the animal or human tissue can not only go deeper but is more effective when going through bones, ligament and tendons. Continuous wave lasers don’t have the ability to drive deep through tissue because they are always on at the same rate of power and therefore delivery.

A lot of people are asking “What is low level laser LLLT?” So here is a brief explanation.

It’s called cold laser because no heat is generated when the laser beam is pulsed. Another name for it is low level laser LLLT therapy (LLLT). It was discovered that low level pulsed laser penetrated living tissue to a greater depth than continuous wave laser and produced no damaging heat.

In 1967 it was accidentally discovered that LLLT regrows hair and accelerates wound healing in mice. In 1979, Russian cosmonauts discovered that LLLT produced positive healing effects on human tissue.

Since then much research has taken place to show that low level laser LLLT activates the mitochondria of damaged cells to produce increased cellular energy (ATP), decreased inflammation and increased micro-circulation. These effects combine to produce the healing we observe while at the same time producing no adverse side effects.

How lllt work?

How Do low level laser LLLT Work?-There are many different types of cold laser devices. Many used by clinicians are often the size of a flashlight. and may be connected to a larger unit (computer), or may be a handheld device. The laser may be placed directly over the injured area for 30 seconds to several minutes depending on the size of the area being treated and the dose provided by the cold laser unit. Some may move the laser around, move the joint or muscle during treatment, and some may remain in contact without movement of either unit or patient.

During treatment the non-thermal photons of light that are emitted from the laser and pass through the skin layers (the dermis, epidermis, and the subcutaneous tissue or tissue fat under the skin). This light has the ability to penetrate 2 to 5 centimeters below the skin. Once the light energy passes through the layers of skin and reaches the target area, it is absorbed and interacts with the light sensitive elements in the cell, such as the mitochondria. This process can be compared to photosynthesis in plants – sunlight is absorbed by plants, which is then converted to usable energy so that the plant can grow. The mitochondria absorbs the light, which is used to make energy in the form of ATP.

When cells absorb this light energy, it also initiates a series of events that is theorized to eventually result in healing damaged or injured tissue. Some of these events include increasing the movement of immune cells from the blood vessels, into the area of tissue injury. Increase lymphatic flow and drainage, as well as increased blood flow and drainage can occur. In short, laser therapy can assist in reduction of pain, inflammation, edema and an overall reduction in healing time by increasing intracellular metabolism.

How low level laser LLLT Work?-low level laser LLLT—also called low-level or near-infrared laser therapy—uses low-level light energy to treat a wide array of conditions, ranging from sprains, tendinitis, back pain, and arthritis to temporomandibular disorder (chronic jaw pain), carpal tunnel syndrome, diabetic neuropathy, fibromyalgia, and leg ulcers. Used by some physical therapists, physiatrists, chiropractors, and occupational therapists, it is different from conventional “hot” laser surgery. The FDA has cleared more than two dozen cold laser devices, including some sold on the Internet for home use, for the temporary relief of pain, which means only that they are safe, not necessarily that they are effective. The therapy is noninvasive and painless; no serious side effects have been reported.

It’s not clear how low level laser LLLT might work, but it may have anti-inflammatory effects, help repair connective and other tissues, and release pain-relieving endorphins.

Overall, though, it’s hard to draw firm conclusions because many of the studies have been small, short in duration, and not well-controlled. Plus, there are differences in the devices and how the treatment is delivered in terms of dosage, wavelength, duration, where on the body it is applied, and technique. low level laser LLLT may work for one condition but not another, and only in some people—and their effect may depend on the degree of injury.

Though low level laser LLLT may provide some temporary relief for some types of pain, it may not affect the underlying ailment or be any more effective than other types of heat delivery,in their coverage policies, both Cigna and Aetna note that there’s little or no conclusive evidence that low level laser LLLT is effective for anything. Even in research suggesting some promise for the therapy in treating conditions such as carpal tunnel syndrome, chronic neck or back pain, and diabetic ulcers, the evidence has been limited and conflicting, and it’s hard to know how much any apparent benefit is due to a placebo effect.

Bottom line: If you have tendinitis or other musculoskeletal pain, work with a physical therapist, who may use low level laser LLLT among other treatments. Some health insurance policies cover it when it’s provided by a health-care professional for certain ailments. Whether it adds much to the benefits of traditional physical therapy—ice, heat, exercise, and massage, for example—is unclear. Lastly, don’t count on getting the same benefit from self-treatment as you might from having it done by an experienced, trained professional. Home devices, which can cost hundreds of dollars and are often touted as a cure for nearly everything, may not have the power to do much of anything.

Review of Literature on Low-low level laser LLLT Benefits for Nonpharmacological Pain Control in Chronic Pain and Osteoarthritis.

Introduction • Low-low level laser LLLT (LLLT) is a form of light therapy that triggers biochemical changes within cells. Photons are absorbed by cellular photoreceptors, triggering chemical alterations and potential biochemical benefits to the human body. LLLT has been used in pain management for years and is also known as cold laser therapy, which uses low-frequency continuous laser of typically 600 to 1000 nm wavelength for pain reduction and healing stimulation. Many studies have demonstrated analgesic and anti-inflammatory effects provided by photobiomodulation in both experimental and clinical trials. Objective • The purpose of this research article was to present a summary of the possible pain management benefits of LLLT. Results • In cold laser therapy, coherent light of wavelength 600 to 1000 nm is applied to an area of concern with hope for photo-stimulating the tissues in a way that promotes and accelerates healing. This is evidenced by the similarity in absorption spectra between oxidized cytochrome c oxidase and action spectra from biological responses to light. LLLT, using the properties of coherent light, has been seen to produce pain relief and fibroblastic regeneration in clinical trials and laboratory experiments. LLLT has also been seen to significantly reduce pain in the acute setting; it is proposed that LLLT is able to reduce pain by lowering the level of biochemical markers and oxidative stress, and the formation of edema and hemorrhage. Many studies have demonstrated analgesic and anti-inflammatory effects provided by photobiomodulation in both experimental and clinical trials. Conclusion • Based on current research, the utilization of LLLT for pain management and osteoarthritic conditions may be a complementary strategy used in clinical practice to provide symptom management for patients suffering from osteoarthritis and chronic pain.
